Background of the Study
Adolescent health services play a critical role in addressing the unique health challenges faced by young people. In Rivers State, Nigeria, these services encompass a wide range of interventions including reproductive health education, mental health support, and prevention of risky behaviors such as substance abuse (Uche, 2023). Given the transitional nature of adolescence, timely and effective health services can influence lifelong health outcomes by promoting healthy behaviors and reducing risk factors. Comprehensive adolescent health services are designed to provide accessible, youth-friendly care that addresses issues such as sexually transmitted infections (STIs), nutritional deficiencies, and mental health challenges. However, in Rivers State, barriers such as stigma, limited service coverage, and a lack of trained personnel often undermine the effectiveness of these services. Studies have shown that well-designed adolescent health programs can lead to significant improvements in health outcomes, yet their impact remains variable across different settings (Adebayo, 2024). This study aims to appraise the effectiveness of adolescent health services in reducing health risks in Rivers State, identifying both successes and challenges to inform future interventions (Ibrahim, 2025).
Statement of the Problem
Despite the availability of adolescent health services in Rivers State, many young people continue to face significant health risks due to insufficient access and utilization of these services. Factors such as stigma, lack of awareness, and inadequate youth-friendly services contribute to underutilization. As a result, issues such as high rates of STIs, substance abuse, and mental health disorders persist among adolescents. The gap between the intended benefits of adolescent health services and the actual health outcomes highlights the need for a comprehensive evaluation of these programs. Without targeted improvements, the potential to mitigate health risks among youths will remain unfulfilled, perpetuating negative health trajectories into adulthood (Chukwuma, 2023).

Scope and Limitations of the Study
The study will focus on adolescent populations (aged 13–19) in selected urban and rural areas of Rivers State. Data will be collected through surveys, focus group discussions, and interviews with service providers. Limitations include potential self-report bias and variability in service quality.
